Patient E: What can I do to give me an overall slimmer face? I would also like the sagging areas to be lifted. Can it all be done in the same setting?

The answer to the question posed above is - The Slim Facelift! 

It is also a common to hear patients having fillers placed to augment the cheeks or along the nasolabial folds, yet most of the time, these fillers might be excessive and not helping to achieve a slimmer look.

If fillers alone were chosen to lift your face, the result will definitely look nice, BUT it also comes with a price - where your face will inadvertently look more puffed up. This might be good in cases where volume loss is significant. However, in most Asian women, good volume is present and just needs to be repositioned to the right areas.

Steps To The Slim Facelift

(1) Complete Facial Assessment
A good treatment would usually start off with the right diagnosis, in our case, it is about being able to see which areas when improved, can give you significant results. Your doctor should be able to point this out for you before discussing any treatment options.

(2) Goal Directed Hyalase

Post Laser Scab Care

Patient: How do I take care of scabs that form after laser treatments and surgery?

Scabs typically form whenever the optimal power is selected for certain lasers and is common and they signify a phase where your skin is raw and just starting to heal up. They scab whenever you want certain results such as
  • Significant Lightening of Pigmentation
  • Removal of Moles
  • Resurfacing Lasers
  • Removal of Red Vessel Spots

Scabs typically fall off in 3-7 days depending on your skin and treatment protocol. You would want to make sure you get the best scab care such that you reap the optimal results from your laser treatment!

Steps to Advanced Scab Care Post Laser

Step 1: Mechanical Protection
Prevention of further mechanical damage.
Avoidance of hot water, abrasives, microbeads, rough towels, mechanical friction, picking over the scab
 This is pertinent to allow your raw skin to heal up well.
Additional trauma can result in unintended damge and may even cause scarring! 

Step 2: Moisturisation
Be generous with moisturisation.
Moisturise every hour for the first 48 hours post treatment and scale it down to 5x a day for 5 more days thereafter. 
Moisturisation is key to give sufficient moisture for skin to soak up and heal as it is usually dry post treatment.
Use of RAI Post Laser Intensive Recovery Serum before your moisturiser is highly recommended for decreasing inflammation and decreased risk of PIH. 

Step 3: UV Protection
 Use an umbrella for 1 month post laser treatment whenever going out. 
Sun block is typically applied in insufficient quantities by almost everyone! If you are diligent in applying 3 x 50 cent coin worth of sun block over the face every 80 minutes, then you will be getting the intended SPF 50 as stated on the sunblock. Sun block can typically be applied 48 hours post laser treatment.
If you are sitting next to a window, try to shift your daily activities to be away from the window where UV-A easily penetrates. UV-A makes up 95% of UV rays from the sun and gives an insidious insult to our skin, especially when it is healing. UV exposure can lead to PIH which will compromise the intended laser results. 

Step 4: Treating Unintended Pigment Formation
Let your doctor know early if this occurs and treatment can be instituted to reverse unintended pigment formation. 
Risk of this developing can be decreased with RAI Post Laser Intensive Recovery Serum and with proper care, this can be avoided.

The double chin is a common problem in both men and women and takes away definition from our lower face. It is an area where fat accumulates and there is pretty much nothing we can do about it. A rigid programme of exercise could help but a mere bit.

Aging of the lips has been described in my prevous post. 

Now, we shall now focus on what we can actually do to achieve the end result - Luscious Lips! 

If the main defect is on the red lips, dermal fillers specific to lips are a good option. However, it is important to make sure that other areas have been addressed such as the teeth, upper white lip, skin quality before jumping to lip fillers. 

Steps to Luscious Lips

1. Consultation & Evaluation​
- A thorough evaluation should be conducted by your doctor before the procedure to determine which areas will give you the best results.
- If other aging mechanisms are present, the use of other modalities should be considered. 
- Failure to do so will usually result in a duck lip where fillers are placed in the wrong place, giving a poor result. 

2. Numbing Preprocedure
- Numbing cream is applied to achieve comfort for the actual procedure.

3. Luscious Lips Filler Procedure

Fat Grafting or Fillers

Patient: Dr Sin Yong, I want to look younger. My friends suggested that I go ahead for fat grafting to improve me facial volume.

Is that the best option for me?

Fat Grafting has been around since the 1900s and has progressed to allow for improvement of volume in multiple regions apart from just the face. Think about buttock augmentation, breast augmentation, etc etc

Dermal Fillers used to be bovine collagen in the 1970s and has went on to include hyaluronic acid fillers since the 2000s, a natural progression of technology and research. 

Here, let's examine the potential advantages of each!

Fillers

Fat Grafting

  • Possibility of dissolving if dislike results
  • Collagen stimulating agents for rejuvenation effect 
  • Variable hardness to give strong or soft contour depending on region intending to improve

What does this mean for patients?

As a patient, you would need to consider the following factors
  1. Down Time- Fat grafting will always involve a surgical procedure to harvest the fat, whereby post procedure, there will always be some swelling and bruising over the donor site. Fillers are pre manufactured and ready to use in sterile syringes.
  2. Fall Back Plan - The question to ask here is whether you are certain of the end result. If you have tried fillers before and like the resultant look, you might be a good candidate for fat grafting if you seek permanent long term results. Conversely, if you have not seen a possible end result before, jumping straight to fat grafting might be a huge risk as you will have to bear the end result as longevity is relatively permanent.
  3. Region to be treated - Certain areas such as the chin, jawline and nose will require a filling with increased hardness and stronger lifting effect. In such regions, fillers with strong lift are superior to fat grafting to provide the ideal result.
  4. Are you ready? - Ask yourself this over and over again before going for fat grafting, as this is something that is minimally reversible. It can dramatically improve your looks permanently, but also carries a possibility of permanently changing your facial contour in a way you might not like. 

Dr Sin Yong Recommendations

Depending on the area treated, the solution might not always be to fill the area with fillers. Take dark eye circles for example, we are always looking at a possibility of increased pigmentation, increased vascularity, or lack of volume. Using fat grafting or fillers as one size fit all treatment will not produce the superior results compared with using multiple modalities to target the root cause of the problem, which might be increased blood vessels or pigments.

Goal Directed Hyalase

Goal directed hyalase targets hyaluronic acid fillers that requires dissolving.

Common regions for goal directed hyalase include:

Hair Loss In Women

A woman's hair allows her to create her beauty and sexuality. Yet, hair loss in women is common and ranges from a variety of causes. The most common is that of female pattern hair loss which starts with a gradual hair thinning/ shedding where family history can play a role. It is normal to lose between 50-100 strands of hair per day and can increase with age. However, increasing number of hair shed during showering or hair dressing should prompt you to seek treatment. The frontal hair line may or may not be affected.

A consideration of other causes that could result in hair loss is pertinent such that treatment can be targeted. Basic blood tests that aim to replace iron, zinc and rule out concurrent thyroid or autoimmune disease can be considered. 

Needless to say, the psychological impact can be severe and hence needs to be treated promptly.
What can we do to arrest the ongoing hair loss? 

Steps To Arresting Loss of Hair

(1) Diagnosis
A diagnosis should be achieved using a combination of presenting history, physical examination and relevant blood tests. Other diagnoses such as telogen effluvium, alopecia arreata should also be entertained.

(2) Topical Therapy
Topical Minoxidil 2-5% is encouraged as a scalp shampoo to prolong the the growth phase of the scalp hair.

Androgenetic alopecia is the  most common form of hair loss. Excessive androgenic hormone - dihydrotestosterone, and genetics, are implicated in progressive miniaturisation of hair follicles.

This ultimately results in conversion of thicker terminal hair into thinner vellus hair. Insufficient flow of blood to the scalp and dysregulation of inflammatory cytokines which promotes microinflammation also contributes to its pathophysiology.

If you are suffering from loss of hair, you are not alone. ​This condition can affects close to 60% of Asians and up to 80% of Caucasians.
Our hair contributes to our self image and identity, and society perceives dense and strong hair as youthful, healthiness, beauty and success. 

Loss of hair results in altered appearance which causes psychosocial distress with decreased self confidence, self esteem, heightened self consciousness and negatively affects the quality of life of patients.

It is hence, important for this to be addressed!

If the decision here is to start and stop every now and then, this therapy will result in rebound hair losses rather than arresting the loss of hair.

Steps to Photoacoustic Tattoo Removal

(1) Evaluation of Tattoo
This is crucial and will determine how many sessions it may take to clear up the tattoo.
Factors that influence the resultant number of sessions include
(A) Your Skin Color
(B) Location of The Tattoo
(C) Color Pigments of The Tattoo
(D) Amount of Ink In The Tattoo
(E) Any Scarring or Layering Involved

We are looking at a combination of steps to treat this effectively! 

Steps To Resolving Rosacea

(1) Avoidance of Trigger Factors
This includes all of the above and also encompasses the regular use of sun protection.

Post inflammatory hyperpigmentation (PIH) is an acquired form of hyperpigmentation that results from either injury or cutaneous inflamation, distributed over the region of causative inflammation.

Common causes include that of acne, accidental injury and burns. 

Post inflammatory hyperpigmentation worsens with sun exposure or recurrence of inflammation. These pigments can be both superficial or deep within the skin.
Is there a way to treat PIH effectively? 
The answer to it is to use a combination of treatments. 

Steps To Resolving PIH

(1) Removal of triggers to inflammation 
This can be as simple as putting a stop to scratching the affected area. Minimising inflammation to the area prevents the area from progression to more areas which can develop PIH. 

(2) Sun protection and sun avoidance
Avoidance of excessive UV lights helps in preventing the production of more pigments. Regular use of susn block in recommended. 

(3) Laser Therapy 
A combination of both the Pigment Removal Laser and the R2 Glow Laser is crucial to target both pigments and underlying inflammation, reaching a optimum speed of improvement for the areas of PIH affected. 

(4) Post laser care
Usage of skin lightening agents can be employed. The best choice will be the use of one that calms the inflammed areas down and decrease the production of pigments at the same time.

Stopping Acne Flares

Acne Vulgaris is a chronic inflammatory skin disease of the pilosebaceous unit that affects about 85% of adolescents. It is characterised by comedones, papules, pustules, nodules and cysts which give it a spectrum of severity.

Yet, only about 40% of people affected seek medical help. ­­The cycle of increase in sebaceuous gland activity, coupled with abnormal follicular differentiation with an increase in keratinisation which in turn predisposes to microbial colonisation leading to increased inflammation propagates the spread of acne. 

Other contributing factors include:
  • Hormonal disturbances from medications
  • Family history
  • Comedogenic products
  • Dietary intake of high glycemic or dairy food which elevates IGF-1 levels and in turn cause an increased sebum production and follicular keratinisation

What can we do to stop Acne Vulgaris?
​The answer lies in breaking the cycle of acne.

I would recommend the following steps to break the cycle effectively.
Other additional steps maybe added for specific skin types.

(1) Thorough Evaluation of Skin Care Regime 
These are the products that we use on our face daily and forms the foundation in our battle against acne. Any comedogenic products should be removed and the texture and consistency of products should be evaluated.

(2) Adjuvant Laser Therapy
A combination of R2 Glow Laser and Pigmentation Removal Laser aims to achieve the following results.
(A) Decreased Inflammation (B) Decreased Bacteria Counts (C) Lightening of Pigmentation & Redness

(3) Chemical Peel / Hydrafacial Microdermabrasion
Chemical peels help to remove excessive dead skin cells which can predispose to clogging of sebaceous ducts which in turn results in comedone formation. 

** You should always have your condition evaluated by a trained doctor to ascertain the diagnosis before commencing any form of treatment. 

How do I know how severe is my acne?
Grading To Acne

(1) Clear
No lesions to barely noticeable ones; very few scattered comedones and papules

(2) Almost clear
Hardly visible from 2.5 meters away; a few scattered comedones and a few small papules; and very few pustules, comedones, and papules

(3) Mild
Easily recognizable; less than half of the affected area is involved; many comedones, papules, and pustules

(4) Moderate
More than half of the affected area is involved; numerous comedones, papules, and pustules

(5) Severe
Entire area is involved; covered with comedones, numerous pustules and papules, a few nodules and cysts

(6) Very severe
Highly inflammatory acne covering the affected area, nodules and cysts present

Do I need to be treated?
Or should I let time pass by and hopefully it gets better?


Like any disease, it is always better to treat it early before it spreads. 
Acne destroys our self confidence, takes away our self esteem and burdens our psychological health, creating a depressed self.

Controlling Melasma

​Melasma is an acquired pigmentary condition that tends to be long term, with patients commonly falling into relapse. It tends to affect women in their 30s and 40s and can occur more frequently in child bearing women. Men can also be affected as well. Contributing factors are abound such as long term ultraviolet exposure, genetics contribution, oral contraceptive use, hormonal fluctuations, thyroid disorders and use of irritant cosmetics. 
What can we do to control it?
The answer lies in understanding Melasma, arresting the aggravating factors, then rebalancing the skin profile!
Arresting The Aggravating Factors
Avoid UV radiation 
Use of broad spectrum sunscreen can achieve up to 26% improvement as early as 3 months. Regular application of broad spectrum sun block can reduce melasma incidence by up to 90% and decrease its intensity by 50%.

Hormones 
Increase in pregnancy hormones or use of oral contraceptives can trigger melasma. 

Avoid over-exposure to heat 
Professions such as bakers who are exposed to oven can have aggravation. 
 
Avoid exposure to high light intensities
P
rofessions such as dentists are more resistant to treatment of melasma. Visible light can induce hyperpigmentation via opsin 3 which is a specific sensor in melasma. Sunscreens containing iron oxide confer protection against these visible light of shorter wavelengths.
 
Avoid use of photosensitizing cosmetics
Found in a range of cosmetics where they can aggravate melasma.

Subcision & Scarring

Scarring that results from acne can disfigure and cause a lack of self esteem. 

The scars that we will look into are those that are of a contour defect, meaning to say no amount of make up can actually cover it up. These are different from darkening or redness that can happen after a pimple has popped!

These scars can be deep and narrow - the ice pick type, or they can also be wide yet shallow, involving a larger area - rolling type. As these scars are treated, gradually when they become more and more shallow and involve a smaller area, they can present like box car type scars. 

Let's focus on rolling scars today!
Is there a way for us to treat rolling scars well?
The answer lies in using Subcision!

Subcision, is essentially a technique used to break the deep scars fibres that hold the scar down, thereby allowing the scarred surface to rise up.
